PLEASE NOTE: Completion of this form does NOT confirm your field trip. A representative from HSMO's Education Department will contact you to confirm all details.

At this time, Education Programs take place at HSMO's Macklind Ave. Headquarters ONLY. 1201 Macklind Ave., St. Louis, MO 63011 (Across from the Zoo and next to the Science Center) 90 minute classes are $5/per child or $50 minimun for the group.
